Admission conditions

 

At the appointment, the patient’s caregiver will be informed that the scheduled admission will be carried out in the absence of symptoms of respiratory illness or other acute illness, on the date of admission.

If there are any signs of respiratory illness or other acute illness, the patient and/or caregiver will be encouraged to inform the admitting physician/family physician and to remain at home under the supervision of the family physician, who makes the decision regarding conduct, depending on the determined situation.

In these cases, hospitalization is postponed with a later appointment after recovery, through information and coordination with the Republican Rehabilitation Center for Children.

PROGRAM ADMITION

Required documents

The patient (family) eligible for rehabilitation therapy presents himself at the Center on the scheduled date and time with the necessary equipment and the necessary documents for admission:

  1. The admition form (form no. 027/e), issued by the family doctor or the consultant, which contains detailed information from the Child Development Record (form no. 112/e), including: perinatal history, dates of clinical examinations, paraclinical (neurosonography, CT, MRI, radiological examination, etc.) and laboratory (the result of the summary examination of blood, urine, carried out in the last 10 days) to which is attached:

1) The certificate about the epidemiological condition issued by the family doctor;

2) Copy of the child’s birth certificate;

3) Copy of the medical report for the child with the established degree of disability from childhood to the age of 18 (form No.- 080/e) – if present;

4) Results of investigations, consultations, conclusions regarding the health status of the child – if present.

5) The informed consent signed by one of the parents/legal guardians, upon admission;

6) Other documents imposed by the epidemiological situation in the country or by order of the Ministry of Health or the Commission for Exceptional Situations – if necessary.

The methodical instruction for reffering patients for rehabilitation treatment to the Republican Rehabilitation Center for Children

 

  1. Children with neuro-motor disorders, aged from 0 months to 18 years, from all the administrative territories of the Republic are reffered to rehabilitation.
  2. The basic principle in the selection of children at risk of developing a disability or with an established diagnosis for rehabilitation treatment in the Center is the presence of amelioration or healing resources, as a result of the rehabilitation process.
  3. Sending children to rehabilitation in the Center is carried out in a scheduled manner, according to the waiting list, confirmed at the unique contact number:

  1. The decision regarding the referral of patients to rehabilitation treatment in the Center is received by the Medical Consultative Council (CMC) of the Center for Family Physicians (CMF), based on the information presented by the family doctor and/or other specialist/consultant.
  2. When reffering the child to the Center, a referral extract (Form 027/e) signed by the CMC members of the CMF from the patient’s place of residence is completed.
  3. Children of pre-school age and those of school age, who do not need to be fully assisted in their daily care routine, during the treatment in the Center are accompanied by one of the parents/guardians, to whom the family doctor issues medical leave for the duration of the treatment, in accordance with the provisions the approved normative acts.
  4. The accompanying parent or guardian of the child, when hospitalized with the child in the Center in the inpatient section, benefits from accommodation and feeding, free of charge.
  5. In the child’s development chart (Form no. 112/e), data regarding the referral of the patient for treatment are mandatory.
  6. The data regarding the patients registration reffered for treatment in the center are entered into the Register, which includes: no. d/b; name, first name of the child, date, month and year of birth, home address, no. by phone; refferal diagnosis; degree of disability; treatment period; ID card number and parent/guardian signature. The respective register is kept in the institution for no less than 3 years.

Discharge procedure

One-two days before discharge, the day and time of discharge will be announced, so that the family can prepare for departure and notify a close person about your discharge.

Upon discharge from the Center, a Referral Statement (Form 027/e) is issued, in which the results of the investigations and the administered treatment, as well as recommendations on the conduct of the case in outpatient settings (the patient is discharged only in the presence of the doctor) are entered .

      Important! Don’t forget to notify the family or next of kin about your discharge. If you are traveling independently, keep an adequate reserve of finance to cover the expenses incurred on the return journey from the Center to your place of live.

Recommendation

When the patient is addmited to the Inpatient Department, please have a copy of the prescription or the packaging to know the doses of the medication the patients are taking. A hospital discharge note or the last investigations the patient have done are things needed to be showen to the doctor.

Security

It is prohibited:

  • The use of mobile phones around patient treatment areas.
  • The use of mobile phones with camera and video without the consent of the medical staff.

We remind you that the hospital does not assume responsibility for high-value items such as: important documents, money, jewelry and other personal belongings. Valuables are NOT to be kept with you in the hospital! Patients are welcome to send all valuables home or entrust them to relatives for safekeeping. It is recommended that patients keep only small amounts of money sufficient to cover daily expenses and an adequate reserve for the return journey from the Center home.

         Important! Don’t forget to tell the Center staff the name, surname, address and contact number of your next of kin.

Visiting schedule and rules

 

Visits (to hospitalized patients) by other people are carried out in open spaces (hallway, park around the institution). If necessary, the number of visitors is limited for security purposes and to avoid overcrowding in the Center. Children accompanying visitors must be supervised by an adult at all times.

If you are asked to leave the ward or hospital with a reason, please comply with the given requirement.

It is also mandatory to follow the advice and rules of the medical staff:

  • Maintain personal hygiene and comply with the internal regulations of the hospital.
  • Do not visit if you have a cold, an infectious disease or are unwell.
  • Do not bring alcoholic beverages, drugs, sharp objects, weapons into the Center premises.
  • Do not facilitate in any way the unauthorized leaving of the Center by patients.
